The Balance 2500 plan by Blue Shield of CA contains an annual deductible of $2,500. The out-of-pocket limit is $7,500, and there is no lifetime maximum. Blue Shield covers 100% of the cost for preventive services, and members make a fixed $30 copay for office visits (applicable before the deductible). This plan also provides coverage for chiropractic care and acupuncture, at a 50% coinsurance. Prescription drug coverage is for both generic and brand name medications. For generic drugs, members make a $10 copayment ; for brand name drugs, there is a separate deductible of $500. After this deductible has been met , members are responsible for a $35 copay.
