The Active Start Plan 25 Generic Rx by Blue Shield of CA contains a $0 annual deductible and a $6,000 out-of-pocket limit. Lifetime benefits for this plan are unlimited.  For office visits, members are responsible for a $25 copay. For other medical services, members pay 40% of the negotiated rate, with the exception of preventive care. Blue Shield covers 100% of the cost for preventive services.
