The BCBS of GA Smartsense Generic Rx 3500 POS plan contains an annual deductible of $3,500, with an out-of-pocket limit of $6,500.  Members pay a $30 co-payment per calendar year for the first three doctor visits (not subject to the deductible). After three visits, once the deductible is met, members are responsible for 30% of the negotiated rate. This plan provides generic-only prescription coverage, with no separate deductible.
