CURRENT MEMBERS:
The Smartsense Generic Rx 5000 plan, by Anthem CO, contains a $5,000 annual deductible and a $7,500 out-of-pocket maximum. Immediate benefits are provided for the first three visits to a doctor: the deductible is waived, and insured members make a low $30 co-payment for each visit. After three visits, and when the deductible has been met, members pay 30% of the negotiated rate. Prescription drug coverage under this plan is for generic drugs only.
