The Smartsense Generic Rx 500 plan, offered by Anthem Blue Cross Blue Shield of Colorado, consists of a $500 annual deductible and a $3,000 out-of-pocket maximum. Immediate benefits are provided for the first three visits to a doctor: with the deductible waived, insured members make a low $30 co-payment for each visit. After three visits, and when the deductible has been met, members pay 30% of the negotiated rate. Prescription drug coverage for this plan is for generic drugs only. There is no separate deductible, and members make either a $15 co-payment,  or pay 40% of the cost (whichever amount is greater).
