The Smartsense Generic Rx 1500 plan, by Anthem Blue Cross Blue Shield of Colorado, consists of a $1,500 annual deductible and a $4,000 out-of-pocket limit. Immediate benefits are provided for the first three visits to a doctor: the deductible is waived, and insured members make a $30 co-payment for each visit. After three visits, and when the deductible has been reached, members pay 30% of the negotiated rate. Prescription drug coverage for this plan is for generic drugs only. There is no separate deductible, and members make either a $15 co-payment, or pay 40% of the cost (whichever amount is greater).
