CURRENT MEMBERS:
The annual deductible for the Smartsense Full Rx 2500 plan by Anthem CO is $2,500; The annual out-of-pocket maximum is $5,000. For doctor’s office visits, Anthem allows 3 visits with the deductible waived at a low $30 co-payment. After 3 visits, and once the deductible has been satisfied, members pay 30% of the negotiated rate. This plan offers comprehensive prescription drug coverage, which includes both generic and brand name drugs (a $500 deductible applies for brand name and specialty drugs).
