CURRENT MEMBERS:
The annual deductible for the Smartsense Full Rx 1500 plan by Anthem CO is $1,500; The annual out-of-pocket limit is $4,000. For doctor’s office visits, Anthem allows 3 visits with the deductible waived at a $30 co-payment.  After 3 visits and once the deductible has been satisfied, members are responsible for 30% of the negotiated rate. Furthermore, after the deductible is met, the same coinsurance applies for all professional services (such as x-rays, lab work, and surgery), as well as both inpatient and outpatient hospital services. The Smartsense Full Rx 1500 plan offers comprehensive prescription drug coverage, which includes both generic and brand name drugs.
