Current Members The Anthem CA Premier 6000 plan has an annual deductible of $6,000, and the individual yearly copayment/coinsurance maximum is $4,500. The out-of-pocket limit for covered services and charges is $10 500. Under this plan, members are provided with unlimited office visits per calendar year (with the deductible waived) at low co-payments, as well as low coinsurance levels for inpatient, outpatient, and emergency room services. To help manage the daunting costs of prescription drugs, this plan has a tier-based system for presciription drug coverage;  varying cost levels for both generic and brand name drugs are provided under this plan.
