The Premier 3500 plan by Anthem Blue Cross of California offers an annual deductible of $3,500, and a copayment/coinsurance maximum is $4,500. The out-of-pocket limit is $8,000. For visits to the doctor’s office, Premier 3500 allows unlimited visits to both primary care physicians and specialists at low co-payments, with the deductible waived. This plan also offers a broad range of preventative benefits; annual physical exams, along with certain preventative screenings are covered before the deductible, once a member has been on the plan for six months.
