Current Members
The Anthem CA CoreGuard plans feature a simple plan design with some of Anthem’s lowest rates. The CoreGuard Plus 2500 plan contains an annual deductible of $2,500, and an individual copayment/coinsurance maximum of $3,500. The out-of-pocket limit for this plan is $6,000. After the deductible is met, Anthem will provide coverage for office visits, along with professional and diagnostic services, at 50% of the negotiated rate. Once the out-of-pocket maximum has been satisfied, Anthem will provide benefits at 100% of the negotiated rate. Prescription drug coverage for this plan is based on a tier-system, which includes both generic and brand name drugs.
