The CoreGuard plans, by Anthem CA, provide affordable protection against significant medical expenses for insured individuals.The annual deductible for this plan is $1,500. The copayment/coinsurance maximum is $3,500, and the out-of-pocket limit is $5,000. For Inpatient and Surgical services, as well as Outpatient and Professional/Diagnostic services, Anthem will cover 50% of the negotiated rate after the deductible is met. Prescription drug coverage is based on a three-tier system that includes both generic and brand name drugs.
