Current Members
The Anthem CA CoreGuard Plus 10000 plan has an annual deductible is $10,000, and a yearly copayment/coinsurance maximum of $0. The out-of-pocket limit for this plan is $10,000. After the deductible is met, Anthem will provide coverage for office visits, along with professional and diagnostic services, at 100% of the negotiated rate. Anthem will also cover 100% of the costs for Inpatient and Outpatient Hospital services after the deductible is met. Prescription drug coverage for this plan is based on a tier-system, which includes both generic and brand name drugs.
