Anthem Blue Cross California ClearProtection Plus 5000

The Anthem CA ClearProtection Plus 5000 plan features two deductibles that work together to help members meet their total out-of-pocket maximum, as well as attain benefits faster for high-cost services, such as surgery and hospitalizations.  The two deductibles are for Inpatient/Surgical services and Outpatient/Professional and Diagnostic Services. The Inpatient/Surgical Deductible is $5,000, and the Outpatient Deductible is $8,500. The Outpatient deductible is equivalent to the out-of- pocket maximum. As noted above, any combination of services from both of these deductibles may be used to satisfy the out-of-pocket maximum. Each calendar year, members are allowed two doctor’s office visits with the deductible waived at a $40 copay. After the deductible is met, no coinsurance is required for the remainder of the year. Prescription drug coverage for this plan is based on a tier system, which includes both generic and brand name drugs.
