The annual deductible for the Smartsense Plus Upgrade Rx 6000 plan is $6,000. There is a $9,500 out-of-pocket limit, and no lifetime maximum. For office visits, the deductible is waived for the first 3 visits per calendar year, at a $30 copay. After 3 visits, and once the deductible has been reached, members pay a 30% coinsurance. Preventive services are covered by Anthem at no additional cost. For other medical services, including x-rays, lab work, surgery, and emergency room care, members are responsible for 30% of the negotiated rate (after the deductible).
