The SelectHMO plan by Anthem Blue Cross of California is the lowest priced HMO plan offered by the carrier. There is no annual deductible, and the out-of-pocket maximum for this plan is $3,000. Lifetime benefits are unlimited, and preventive services are covered 100% by Anthem. For doctor’s office visits, members make a flat $25 copayment. For professional services that are rendered during an office visit, such as x-rays and lab work, there is no additional charge. Prescription drug coverage under this plan includes both generic and brand name medications. There is a $10 copayment for generic drugs, and a separate $500 deductible applies for brand name drugs.
