The Premier 3500 plan by Anthem CA was recently changed, and the information below reflects the benefits available before the recent health care reform measures.If you purchased the plan before March 23, 2010, your plan is considered “Grandfathered”, and the benefits outlined below apply to you. CURRENT MEMBERS:
The Premier 3500 plan by Anthem Blue Cross of California offers $7,000,000 in lifetime benefits per member. The annual deductible for this plan is $3,500, and the copayment/coinsurance maximum is $4,500. The out-of-pocket limit is $8,000. For visits to the doctor’s office, Premier 3500 allows unlimited visits to both primary care physicians and specialists at low co-payments, with the deductible waived. This plan also offers a broad range of preventative benefits; annual physical exams, along with certain preventative screenings are covered before the deductible, once a member has been on the plan for six months.
