Due to the recent health care reform measures, benefits for the Premier 2500 plan by Anthem CA have changed. CURRENT MEMBERS:
The Premier 2500 plan by Anthem Blue Cross of California provides $7,000,000 per member in lifetime benefits. The annual deductible is $2,500, and the yearly copayment/coinsurance maximum is $4,500.  The out-of-pocket limit for this plan is $7,000. Regarding office visits, members are offered unlimited visits with primary care physicians or specialists at low, predictable copays, with the deductible waived. Members make a $30 co-payment for physician visits, and a $50 co-payment for a specialist. This plan contains a tier system-based prescription drug package, with varying cost levels for generic and brand name drugs.
