The Lumenos HSA 1500 plan offers traditional health care benefits that can be paired with a Health Savings Account (HSA) for more flexibility and potential tax advantages . This plan features a $1,500 deductible, $5,000 out-of-pocket limit, and no lifetime maximum.  For preventive services, Anthem CA covers 100% of the negotiated rate. For other medical services, such as office visits, as well as professional and diagnostic services (x-rays, lab work, anesthesia, surgery, etc.), members are responsible for a 30% coinsurance, after the deductible has been met. Prescription drug coverage for this plan includes both generic and brand name drugs.
