The Anthem CA HMO Saver is a mid-priced HMO plan that includes access to Anthem’s entire HMO network (over 37,000 doctors). This plan contains an annual deductible of $1,500, which applies to hospital and emergency services. The yearly out-of-pocket limit for this plan is $3,000, and there is no lifetime maximum. There is a $10 copayment for doctor’s office visits (no deductible applies), and preventive services are covered at no additional cost.
