This innovation helps employers in selected SIC codes to:
• Do something good for their seasonal workers
• Take advantage of business tax savings
• Be part of the solution for our state’s uninsured population
What You Need to Know:
• Available October 1, 2004
• For new groups as well as existing groups on their anniversary renewals
• For qualified groups in these agricultural SIC codes only:
Crops (0111-0191) - Livestock (0211-0291) - Support and Management (0711-0762)
• Current group participation and eligibility requirements apply
• Must have zero-month probation period
What Employers Need to Do:
• Write in the company’s SIC code in the Employer Information section of the Employer Application (#5773).
• Complete the Conditions of Enrollment form (#10080).This new form is available on the Agent Web site or
through Supply.
• When seasonal workers arrive, make sure they fill out the Employee Application (#3345) and write the word “Seasonal” in the box where it says “Occupation/Job Title.”
• When seasonal workers leave, immediately fill out the Information Change Form (#5063) and mail or fax to Blue Cross.
