FirstChoice PPO
With FirstChoice PPO, Health Net pays 100 percent of the first $500 of covered services from participating providers...per member per year! That means members don't start paying copayments, coinsurance or deductibles until our $500 has been spent. Unlike other first dollar plans, there are no excessive restrictions or quarterly requirements. It's a true first dollar benefit designed to meet your client's everyday health care needs.

How FirstChoice PPO Works
It's simple. At the start of the benefit year, subscribers and each covered family member have $500 in immediately available funds. They visit a doctor or hospital. The doctor or facility bills Health Net. Health Net covers the first $500 at 100%. After the $500 per family member has been spent, his or her covered medical services are subject to the plan deductible. This means eligible members pay 100% until the deductible is met. Afterwards, they pay a fixed dollar copayment or a coinsurance percentage.

For services covered under the benefit plan and up to the available balance. The $500 in FirstChoice PPO funds cannot be shared between one family member and another and cannot be applied to the prescription drug benefit.

SmartChoice HSA
SmartChoice HSA is a Health Savings Account (HSA) qualified health insurance plan. While individuals must open up an HSA to take advantage of the tax-related savings, anyone can apply for the SmartChoice HSA plan.

Key HSA features:
HSA funds can be used without penalty for "qualified medical expenses" that include medical care and prescriptions, as well as plan deductibles, copayments and coinsurance amounts.2

Individual contributions are tax-deductible.

The funds can be invested and earnings are tax-free.

There is no time limit for using the funds, they rollover from year-to-year.
