Most people set up their billing options during the application and most claims are run through your in-network doctors. There are, however, a few times where you will need to send it a claims form or you might want to change your billing options. If necessary, then you can use the forms below. Due to the recent health care reforms, the Tonik 1500 plan by Anthem NV is no longer available to new applicants. If you purchased this plan before March 23,2010, then your plan is considered “Grandfathered”, and the benefits outlined below apply to your policy.

The Anthem Blue Cross Blue Shield of Nevada Tonik 1500 Plan, aka the “Calculated Risktaker” plan, covers medical, dental and visual services, and contains the lowest deductible of all three Tonik plans. It features a $1,500 deductible, no additional coinsurance, and unlimited office visits for a low $40 copay. Prescription drug coverage for this plan is for generic drugs only. Current members with the Tonik 1500 Grandfathered plan can scroll down for a summary of benefits, as well as links to member forms and information on Tonik dental services.
