The CoreGuard plans by Anthem CA offer choice and flexibility to individuals, allowing them to customize a plan to suit their needs. One of these plans is the CoreGuard Plus 7500. This plan contains an annual deductible of $7,500, and a copayment/coinsurance maximum of $3,500.  The out-of-pocket limit is $11,000. For Inpatient and Surgical services, as well as Outpatient and Professional/Diagnostic services, Anthem will cover 50% of the negotiated rate after the deductible is met. Prescription drug coverage is based on a three-tier system that contains varying cost levels for both generic and brand name drugs.
