Current Members
The individual deductible for the Anthem CA CoreGuard Plus 750 plan is $750, and the yearly copayment/coinsurance maximum is $3,500. The total out-of-pocket maximum is $4,250. After the deductible is met, coverage is provided for office visits, along with professional and diagnostic services, at 50% of the negotiated rate. Prescription drug coverage includes both generic and brand name drugs; the CoreGuard plans use a special Formulary, a list of safe and cost-effective drugs,  to help members manage these expenses.
