The Anthem Blue Cross of California CoreGuard Plus 3500 has a yearly deductible of $3,500.  The copayment/coinsurance maximum is $3,500, and the out-of-pocket limit is $7,000. For office visits, professional services, and preventative care services, Anthem will cover 50% of the negotiated fee rate after the deductible is met. Furthermore, once the out-of-pocket maximum has been reached, Anthem will cover 100% of the costs for these benefits. For prescription drug coverage, Anthem uses a special Drug Formulary. This Formulary contains a list of safe and cost-effective drugs, and includes both generic and brand name medications.
